What is ALWP?

The Assisted Living Waiver Program (ALWP) helps low-income seniors and disabled adults in California live in assisted living facilities instead of nursing homes. It provides services like help with daily activities, medication management, and meal planning. The program is funded by Medicaid and requires participants to pay a portion of their living costs. Its goal is to improve the quality of life by offering care in a more home-like setting​.

WHO QUALIFIES

> Age and Disability:

Must be at least 21 years old.

Must be eligible for Medi-Cal (California’s Medicaid program).

Must have care needs that require nursing facility services.

> Financial Eligibility:

Must meet Medi-Cal’s income and asset limits. For single individuals, the income limit is typically up to 138% of the federal poverty level (FPL). Asset limits also apply and some maybe exempt.

> Medical and Functional Needs:

Must require a level of care that would typically be provided in a nursing home, such as assistance with activities of daily living (ADLs), medical supervision, and personal care.

HOW TO APPLY

ALWP provide vital support for individuals needing medical care or assistance. Applying for these services involves assessing specific needs, researching providers, contacting them directly, and arranging an assessment. A qualified representative evaluates the patient's needs to create a personalized care plan. Understanding payment options and insurance coverage is essential. Once a care plan is agreed upon, services begin with clear communication and ongoing monitoring to ensure effective care and patient satisfaction. This process ensures individuals receive tailored support while maintaining comfort and independence in their homes.​

Step By Step

(1) Talk to Your Specialist:

  • Discuss your need for more care with representatives.

  • Your rep will determine if you qualify and write a prescription.

(4) Submit Paperwork:

  • Provide the agency with necessary documents, such as medical records and insurance information.

(2) Medical Eligibility:

  • Representative will handle your approval through agency and care channels.

(5) Approval:

  • The agency will work with the state and handle getting your approval

(3) Initial Assessment:

  • Virtual assessment will be done to create individual service plan for Client.

(6) Placement:

  • Once approved we will Coordinate moving you into an Assisted living Facility.
